PURIST: You’ve described the mouth as “one of the body’s most powerful biological gateways.” How does what happens in the mouth connect to whole-body inflammation, brain health and cardiovascular risk?

JONATHAN B. LEVINE: The mouth is the second-largest microbiome in the body, home to over 600 bacterial species. When healthy, it is a protective barrier. When it isn’t, it becomes a portal. We call it “leaky gums,” the oral equivalent of leaky gut. Chronically inflamed, bleeding tissue gives pathogens like Porphyromonas gingivalis a direct route into the bloodstream.

From there, we’ve found oral bacteria embedded in the carotid artery walls of cardiovascular patients and in the amyloid plaques in the brains of those with Alzheimer’s. There are now 57 documented diseases linked to chronic oral inflammation, including diabetes and rheumatoid arthritis. A dental visit is one of the earliest windows we have into whole-body inflammation.

Cleanings and X-rays catch decay and bone loss, but tell you almost nothing about the underlying biology. Saliva is a different story. How does gut-oral dysbiosis—or a microbial imbalance between the mouth and the gut—manifest in practice, and how does that disruption ripple downstream?

The mouth, gut and brain operate as one continuous, bidirectional network. Dysbiosis occurs when the oral microbiome shifts away from healthy bacteria toward an overgrowth of harmful, inflammatory species—usually because the environment has become too acidic.

Every time we swallow, we seed the gut with whatever lives in our mouth. An oral environment dominated by pathogens disrupts downstream gut flora, and those inflammatory molecules can travel through the bloodstream, contributing to digestive issues and low-grade systemic inflammation. It is genuinely bidirectional: Gut imbalance shows up in the mouth, and oral imbalance shows up in the gut.

What are the early, easy-to-miss oral signals that someone’s overall health may be under strain?

Bleeding gums during brushing or flossing is the single most overlooked signal. It isn’t normal; it is inflammation, full stop. Persistent bad breath is another warning sign people routinely dismiss with a mint. Other signals include red or puffy gums, gum recession, slightly loose teeth, unexplained dry mouth, or a noticeable shift in taste. Together, they are often the earliest evidence that thyroid inflammation, autoimmune activity or metabolic strain is in motion elsewhere. The mouth tends to show its hand before bloodwork does.

How do issues like poor sleep, snoring or anxiety trace back to the mouth and jaw?

More often than patients suspect. Our imaging scans map the anatomy from the nose straight through the airway, allowing us to see a deviated septum, a narrow airway or chronic mouth breathing directly. Mouth breathing prevents deep, regenerative stages of sleep, which is deeply intertwined with anxiety, irritability and daytime fatigue.

What are two or three things anyone can start doing at home this week to support their oral-systemic health?

First, stop dismissing bleeding gums. If you see blood, it deserves a conversation with your dentist. Second, brushing for two minutes twice a day and flossing daily actively manages the low-pH conditions that allow harmful bacteria to take over. Third, monitor your sleep.
